Registration
Mandatory team registration for all teams will be held from 4:00 to 9:00 p.m. on Friday, May 22nd,2009 at the Tournament Headquarters located at The Four Points by Sheraton Leominster.
The following items are needed for team registration:
· 2008/09 Certified State/National Team Roster (Original and two copy)
· Medical release forms for each player (download here if you need one)(Do not have to be notarized)
· 2008/09 Player Passes for each player
· Permission to Travel Form (if necessary)
Permission to travel forms are not required for teams from Massachusetts.
Teams may not play without having registered under penalty of forfeiture of games and entry fee.

Points Awarded as Follows:
Win: 6 points
Tie: 3 points
Shutout: 1 point
Loss: 0 point
Each Goal Scored: 1 point (max 3 goals per game)
Red Card: 1 point deducted each
